A teacher questionnaire can make a huge difference when giving birthday, Christmas or Teacher Appreciation gifts. I mean, we’ve all gotten gifts we don’t actually want, right?!

When I was in elementary school, I dreamt of becoming a teacher simply so I would be able to grade papers and create worksheets.
Turns out, I had no interest in the actual teaching part. 😅
Long story short, I never became a teacher, BUT my love for formatting documents never faded with time. I seriously love it. 🤓
Now, after becoming an accidental room mom, I’m putting this weird hobby to work. Room moms deserve pretty + functional printables, especially when first getting to know their child’s teacher.
With this in mind, we’ve put together a free, full-color PDF questionnaire for you to break the ice with your child’s teacher while possibly earning a few brownie points in the process. 😉

Sometimes it’s nice to customize the questions and tailor them to the teacher. For example, if you know your child’s new teacher LOVES cats, but you’re not sure what kind or how many cats she has, you can create a line just for that!
What to Do with Your Completed Teacher Questionnaire
Once the teacher returns the completed questionnaire, you can attach it to your room mom intro letter to parents. I like to include this with my introduction letter because it provides VALUE to the other parents that they can use throughout the year.
As s room parent, I always want to be as helpful as possible. Including a list of all the best gift ideas for the teacher is a great way to start off!
